Data audit - August 2021

In order to keep our records up to date, we will be checking all of our member's data at least two times per year. This is to ensure any communications we may need to send to you by email, whatsapp or text message.
In addition into ensuring your details are up to date, we will also be collecting the following data for the first time:

• Allergies we should know about
  
• Medical conditions we should know in the event that you get injured (this is optional, should you wish not to disclose this)

All members, regardless of their length of time of membership must complete the form below, even if all your details you used when signing up is still up to date.
All information submitted will only be used according to our data protection policy. All information given is confidential.
